Certain EverGuard subscription plans include identity theft insurance benefits designed to help reimburse eligible expenses related to identity theft incidents.
This disclosure explains how that coverage works.
Insurance Provided by Third-Party Insurer
This disclosure is a summary of insurance benefits and does not include all terms, conditions, exclusions, or definitions of the applicable insurance policy.
All benefits are subject to the terms of the insurance policy issued to Enfortra, Inc. A complete copy of the policy is available upon request. If the underlying group policy is terminated, insurance benefits will cease as of the termination date.
Identity theft insurance benefits available through EverGuard are underwritten and administered by American Bankers Insurance Company of Florida, an Assurant company, under group or blanket insurance policies issued to Enfortra, Inc. as the policyholder, for the benefit of eligible EverGuard members enrolled in qualifying plans
EverGuard is not an insurance company, does not issue insurance policies, and does not determine insurance claims. EverGuard does not act as an agent, broker, or representative of the insurer.
Coverage is subject to the terms, conditions, exclusions, and limitations of the applicable insurance policy.
Coverage Limits
Coverage applies only while the member is actively enrolled in a qualifying EverGuard subscription and in good standing.
Eligible EverGuard plans may include identity theft insurance benefits with reimbursement limits such as:
• up to $25,000 for certain restoration expenses
• up to $1,000,000 – $5,000,000 per membership per policy period, depending on plan tier
Actual coverage amounts depend on the policy associated with the subscriber’s plan.
Covered Expenses
Identity theft insurance may reimburse certain expenses related to identity theft restoration, which may include:
• legal fees
• lost wages
• document replacement costs
• fraud resolution expenses
• costs associated with restoring identity records
Coverage may also include additional benefits depending on the policy, such as:
• cash recovery coverage
• cyber extortion reimbursement
• home title fraud recovery expenses
Coverage details, limits, and eligibility vary by policy and plan tier and are governed solely by the applicable insurance policy.

Claims Process
If you believe you are eligible for reimbursement under the insurance policy, you must submit a claim through the designated insurance provider.
The insurer is responsible for:
• evaluating claims
• determining eligibility
• approving or denying reimbursement
• administering policy benefits
EverGuard may assist users by directing them to the appropriate claim process but does not administer or approve claims.
All claim determinations are made solely by the insurer in accordance with the policy terms.
EverGuard does not guarantee that any claim will be approved or reimbursed.
Policy Terms and Exclusions

Coverage may not apply in certain situations, including but not limited to:
• incidents occurring before enrollment
• fraudulent activity committed by the insured individual
• failure to take reasonable steps to protect personal information

Active Membership Requirement
Insurance benefits are available only to individuals who:
• are actively enrolled in a qualifying EverGuard subscription plan
• have an account in good standing
• meet eligibility requirements defined in the policy
Coverage terminates if:
• the subscription is canceled or expires
• payment is not maintained
• the underlying insurance policy is terminated
